• Coordinator Post: Graduation in Social Work
• Nurse:  GNM / B.Sc Nursing
• PRO: Post Graduation
• TB Health Visitor: Intermediate
• Supporting Staff: Matriculation

Age Limit

Applicant Age not more than 40 years

• Search for the Kozhikode Staff Nurse notification
• Download the Application form and take a print out of it
• Fill the Application form and send it to the given address before the last date

General Terms & Conditions 

1. Selection will be based on qualification, experience, and performance in the written test and interview. 
2. The application should be submitted on or before 09/08/2019 at 5 pm 
3. The application  should be sent to " The District Programme Manager", Arogyakeralam, Civil Station, Kozhikode -673020" 
4. The-Candidate should be sent separate application for a separate post 
5. The Candidates are requested to enclose self-attested copies of certificates which - prove age, qualification & experience along with the application. The applicant should [omprtioryiitt email id & contact number. Future communications are only through email. 
6. After verifying the applications and certificates, the qualified candidate will be invited for written ex;m /interview. Time and Venue only be informed through email' The successful candidate will be appointed on contract/daily basis for a period of three months. The District health &'family welfare society, Kozhikode may at its discretion based on the performance and integrity of the successful candidate' may extend the period of contract 
7. The selected candidates will be governed by the rules and regulations of NHM in force from time to time 
8. Ranklist of the candidate will be published in Arogyakeralam Website 
9. Candidates are requested to bring originals of lb proof and certificates prescribed as Per qualification.
10. The  District Health & Family Welfare Society reserves the right to -- cancel atter/modify this notification, if needed, without further notice and without assigning any reason thereof.
11. Health & Family Welfare Society reserves the right to cancel the candidate from the selection list at any stage of posting in the absence of relevant certificates or records.
